This form is required in order to re-enroll your child for the following school year. Please submit this form to our Office Manager via email by the due date in order to secure your child's spot. 

If the form is not submitted by the due date your child's spot will become available to a child on our waitlist.

This form is required in order to enroll your child in our summer program. Once the school year ends (see contract or handbook for school year dates) our summer program begins and runs until the following school year starts in the fall. If you do not register your child for our summer program they will not have a spot over the summer months. Please submit the form to our Office Manager via email by the due date in order to secure a spot for your child for the summer. Please know that we fill up quickly in the summer so we encourage you to submit the form as soon as possible. Your child's spot & schedule is not guaranteed once the form is submitted, but once our Office Manager has confirmed their enrollment with you and secured their spot.

This form is required to be submitted to our Office Manager upon request if you are claiming exemption for any of your child's required vaccinations. 

After volunteering please fill out this form to document your hours. Remember: if you chose to volunteer vs. pay the volunteer fee ($40) you need to volunteer at least 2 hours during the school year.
